Hi another aspect would be to look at the chain of command in identifying who had the information, who performed the patient handover - this where the fault lies... then identify the best team member and method of communicating information to the patient.
Also
Who missed the communication to the departments from the patient - was there verbal or non verbal communication - why it may have been missed..
